Please send our OG Head the following info:

1. Blood type
2. Email Address (make sure it is the one that you check most frequently)
3. Next of Kin contact, name, and relationship to you
4. Dietary Restrictions (e.g. Halal, Vegetarian etc)
5. Medical Allergies (if any)

Please do so ASAP because we need to submit the finalized contact list to the Orientation Adhoc by Friday 7pm (: Thanks!
